Admission Requirements
Section 2 – ADMISSION REQUIREMENTS
2.1.Students applying for admission to CVCITC shall be required of the following:
2.1.1. For New Students
Accomplished form of Parent’s Identity Profile.
a. High School Report Card (Form 138)
b. Certificate of Good Moral Character
c. Photocopy of Birth Certificate
d. 4 copies of latest 1×1 I.D. picture with green background
e. Satisfy other requirements that may be prescribed by the school
2.1.2. For Transferees
They represent the group with Certificate of Good Moral Character from the last school attended
a. Official Transcript of Records/Certification of Grades
b. Honorable Dismissal
c. Certificate of Good Moral Character
d. Photocopy of Birth Certificate
e. 4 copies of latest 1×1 I.D. picture with green background
2.1.3. For Married Students
They are the group with marriage license copy furnished to the Student Affairs Office
a. Photocopy of Marriage Certificate
b. Official Transcript of Records/Certification of Grades (if transferee)
c. Honorable Dismissal (if transferee)
d. High School Report Card
e. Certificate of Good Moral Character
f. 4 copies of latest 1×1 I.D. picture with green background
2.1.4. For Disabled Students
They represent the group of students with simple learning impairments that could not directly affect the flow of classroom conditions. The following requirements are to be submitted to the SSWD Office for enrollment purposes
a. Doctor’s Certificate of Disability
b. Parent’s Consent (Waiver)
c. Teacher’s Approval Form
d. Finance Approval as to Budgetary Requirements
